Many plans let you earn rewards for simple activities like completing a health assessment, tracking your fitness, or staying on top of preventive care. Blue Cross Blue Shield offers up to $100 in gift cards through their Well OnTarget and Blue Points Reward programs. UnitedHealthcare offers rewards programs for both employer-based plans and individual and family plans.
